School year 2008-2009

Spring KK Registration

Wednesday, April 30 ~ 8:00 am – 4:00pm

Thursday, May 1 ~ 8:00 am – 7:pm

To register your student bring with you:

-Certified copy of Birth Certificate (NOT the Hospital Certificate)

-Copy of Social Security Card

-Eye, Ear, and Dental Screening (form available at Health Dept, Dentist, and Pediatrician)

-Immunizations on a Georgia Form (available at Pediatrician or Health Department)

We understand that many children will not have completed their immunizations until their 5^th birthday which may be during the summer. You will have to provide the completed immunization form in order to start school

-TWO (2) Proofs of Residence (i.e. Drivers License, Utility Bill, Lease, etc.) We are looking for something that links your name to the address.

- Address of the Preschool your child is presently attending.

- Phone #’s of people authorized to pick up your children
